Athletic Training Forms for Student-Athletes

All of the following forms must be filled out completely in order to participate in athletics at Arcadia. The Arcadia University Athletic Clearance Form must be completed and signed by a physician or other health professional (Nurse Practitioner or Physician Assistant).
