Community Service Forms for Clubs and Classes
These are the forms that all clubs and service classes are required to fill out and turn back into the Community Service Office for their project. Each club must complete at least one project every semester. The first form (Proposal Form) must be filled out and turned in by October 24, 2012 for clubs during the fall semester. The service project may be done at any time throughout the semester, but the proposal must be submitted by the deadline. The second form (Evaluation Form) must be turned in no more than 2 weeks after the completion of the project.
