To obtain a number, contact the Philadelphia office of the DEA. This is the only Pennsylvania office that processes applications from PAs. The PA must request an application with a mid-level practitioner addendum form. A mid-level prescribing manual is also available. This application must be completed by both the PA and the supervising physician. Cost for the number is $210, and the number is good for three years.
