The following forms are available to download by clicking on the appropriate link. The majority of the forms are necessary documentation required of all graduate students to ensure a successful and timely completion of their respective graduate programs. Please take note of deadline and submission dates to avoid a lapse or delay in completion of your program plan.
Act 48 Course Reporting Request
Application. You must make a formal application to the graduate program upon completion of 9 credits. Please select the appropriate cohort on the Partnerships page. An application must be completed and returned to Arcadia University, Center for Adult and Professional Studies, 450 S. Easton Road, Glenside, Pa 19038. A completed application will include recommendations, official transcripts, essay, and PRAXIS results when applicable.
