Below are graduate forms available for download in PDF and/or Word Document format. Please submit all forms to the Graduate Office, Taylor 200 unless indicated. Praxis Series Test Requirements 2011-2012

Degree Candidacy Forms**
Upon completion of 15 graduate credits (excluding transfer credit) students must make formal application to be a candidate for a degree. Requirements for candidacy are good academic standing and advisers signature. Academic advising appointment is recommended.

Upon completion of certificate program students must submit the form to the Graduate Office for processing. Completion requirements are good academic standing and matriculation in an Arcadia Graduate program.

NOTE: $100 application fee in the form of a money order (payable to the Commonwealth of Pennsylvania - Effective July 1, 2010.
*** The Pennsylvania Department of Education PDE338C form changed to include a gpa qualifying score for Praxis exams. The newest forms have (Revised 3/10) at the bottom of the page. Please check your form to ensure that you have the most current version.
New Qualifying Praxis Scores
***Pennsylvania has recently changed certification requirements for several programs. Please be aware that you must complete the current N-3, K-6, SPED K-12, and Dual Certificate (K-6/SPED K-12) requirements by May 15th, 2013. This means that you must complete all coursework, student teaching and must pass all PRAXIS by the end of Spring 2013 semester. All completed certification applications must be submitted to Mary Kate McNulty by August 1, 2013 at the latest. After August 2013, Pennsylvania will NO LONGER be issuing certificates for the current N-3, K-6, SPED K-12, and Dual certificates (K-6/SPED K-12). Any incomplete coursework, PRAXIS , student teaching or application will result in students being required to apply for the new certificate which will involve taking new coursework.
It is recommended that you meet with your academic advisor to map out your current program and answer any questions you may have.

ESL Certificate Application Instructions
(Include in this application the ESL Request Letter and two PDE forms below)
ESL Request for Letter of Completion
To request a duplicate certificate or name change on a current Pennsylvania State Certification download PDE338D and submit it with a $100 money order (beginning July 1, 2010) payable to the Commonwealth of Pennsylvania to the address listed on the form.
To apply for Level II Instructional Certification download PDE338P and make application through your school district. Instuctions are included with the form.
To apply for an add-on certificate download PDE338G_Add-on and apply directly to Harrisburg. Include Praxis scores, $100 money order payable to the Commonwealth of PA and a copy of your current certification(s). Qualifying add-on certifications are for middle-school and secondary school content only. N-3, K-6 and K-12 content areas require coursework from an approved institution and will not be awarded by the Pennsylvania Department of Education. Students must hold a current and active instructional certificate to qualify for add-on certification.
